Photo / Video Release & Consent Form

 

Jones Hockey Development LLC

 

I, [Participant's Name], hereby consent to Jones Hockey Development, Bryan Jones, its skill coaches and all Jones Hockey Development employees to take video footage and photographs of me during my participation in Jones Hockey Development programs, events, and activities. I understand that these videos and photographs may be used for advertisement and promotional purposes by Jones Hockey Development.

I understand and agree that Jones Hockey Development may use these videos and photographs in various media formats, including but not limited to print materials, website content, social media platforms, and promotional materials.

I waive any right to inspect or approve the finished product(s), including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the video footage and photographs.

I release, discharge, and hold harmless Jones Hockey Development, its skills coaches, Bryan Jones, and all Jones Hockey Development employees from any and all claims, demands, or causes of action that I may have against them arising out of or in connection with the use of the video footage and photographs, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights.

I affirm that I am at least 18 years of age and that I am competent to enter into this agreement. If I am under the age of 18, I affirm that my parent or legal guardian has provided consent on my behalf.

I affirm that by taking part in any Jones Hockey Development event, including but not limited to Private sessions, Team Sessions, Clinics, Camps and Summer hockey programs, that I have to understand this policy.

 

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, PHOTO & VIDEO RELEASE AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. 

By Registering, you have read the above carefully and agree to everything stated. 

I have read this Photo & Video Release / Consent Form and understand that I have given up substantial rights by agreeing to it. I agree to this waiver voluntarily, intending to be legally bound.
